To be fair, there are some really AMAZING people at Optalis. MORE ON THEM BELOW.. BUT there was one particular employee who was clearly an awful person. She left residents in their own waste, left my dad without a blanket several times, soaked briefs, never clean. Walked in one day and he had an entire cup of soup dried all over his shirt. Complaints fell on deaf ears, his last week there the entire management staff left. Her name is Shannon, she is an aid or a CENA but should get out of healthcare. . I do not understand how she has a job. She is there for the paycheck, not to care for the vulnerable. SHE is the reason we transferred my dad out of there. If you see her, keep a close eye on your loved one, you'll know who she is, she starts buzzing around when she knows she isn't treating a patient right. POSITIVES: SARA the Receptionist, always joyful, helpful and kind. LORI is the sweetest, kindest caregiver that we had the pleasure of working with. She is loving and patient and just a wonderful person. We would have taken her with us if we could have. KRISTI C. , Lexi and Shelley P.... AMAZING The entire Activities staff! Wonderful people with huge hearts. If you take your family there, be sure to show up often and PAY ATTENTION. If SHANNON is a caregiver for your family, BEWARE. SHE IS LAZY, UNKIND AND DOES NOT CARE.

My mother was here for rehab for a short period and graduated from therapy. The therapy department is wonderful and we loved her nurse that was consistently present, attentive and so sweet and caring, Lexy. My mother adored her! She wasn’t keen on the food but overall a great place for your loved one to get strong enough to go home.

I can't say enough about Charity Morgan, the Director of Nursing at Optalis. She is the heart and soul of this facility, treating every resident with deep compassion and unwavering commitment. Her leadership is exceptional—she creates a warm, supportive environment for both staff and residents, making sure everyone receives the best care possible. Charity isn’t just great at her job; she’s *perfect* for it. Her positive attitude, genuine kindness, and depth of experience make a world of difference. She leads with strength, grace, and an undeniable passion for nursing. And now, she’s taking her expertise even further by pursuing her master's degree to become a nurse practitioner! If you're looking for a facility where the leadership truly cares, where the residents are treated with dignity, and where excellence is the standard, Optalis is it—thanks to Charity. She is an absolute rock star, and I couldn't recommend this place more highly!

My grandma was there and they were super to her. They took great excellent care of her in her final hours. Especially the Nurses. Great place for your loved one.

I think the director of nursing is the nicest. She is definitely the most qualified woman for the job!
