Please do not place your loved one at this rehab center. We had to spend all day and night with her (up until 11:00 pm when we had to leave) to make sure she was safe and taken care of. The food was absolutely not edible. The care was nonexistent. We spent many nights calling the desk at 1 or 2 in the morning to try to get someone to help our mom after we left at 11:00 pm. We transferred her out of there as soon as we could.
I stayed at Optalis Bloomfield Hills for over 6 weeks and received the best rehab and nursing services ever. I am now able to do things I was unable to do before and the nursing team was so pleasantly welcoming it left a warm spot on my heart forever. I can not speak for anyone else but my rehab experience here was great I met with many residents on the rehab unit with me who seem to also be progressing and happy with the care provided. The nurses are very attentive even if it was their first time working with me they picked up on my care needs almost immediately. They made sure I felt safe…

The food is great I was not expecting such a great meals in a nursing home I wish I would have taken a to go plate. Thank you leadership at Optalis Bloomfield who kept checking in following through to ensure my stay was going well. Shout out to this unbelievable life saving team ❤️ God bless you all and thank you for your services
Please I’m begging you do not send your loved one here. We had to hire a full-time caregiver to sit with our mother because they did not have enough staff. They hire contract workers who don’t care. When I challenged them about their staffing ratio - they said oh the PT is staff- you all know a PT is not changing patients out of their dirty briefs! The care was awful. Non existent. Our mother sat in dirty brief for hours and was told, you rang the bell already this hour you can’t ring it again. She would ask for her water to be closer bc of limited range of motion- they said no it’s right…

She would ask for her water to be closer bc of limited range of motion- they said no it’s right there, you can get it yourself. It was dangerous for our mother to be there. We never felt safe. No follow up or follow through from admin. I have never ever seen care this horrible. Not one resident on my mother’s floor was happy. Every family member was trying to get their loved one out of there. So incredibly rude. The State needs to be called on them.
I want to recognize Cheryl and Jen in Activities. They always have great games and activities !! Most of the staff here is real nice. I also want recognize Samar in Admissions. She is beautiful 🤩 inside and out. She checks up on me. I would recommend this facility for Rehabilitation.
My church member was a resident at optalis Bloomfield Hills her stay was very comforting. When I visited she would say I’m alright here people are good. Housekeeping kept room clean. Versie and Anntonette were very welcoming. May she rest in peace. Great job.
The administration at Optalis Health were so caring & heartfelt. They were very attentive & patient when it came to switching my father’s hospice especially Admissions Director Samar Malouf & Social Worker Director Narmeu Jolash. They both were with me & my mother in my father’s room going through the whole process like they were family. Optalis Health is a very clean & well kept facility. Great Staff. The nurses were so Friendly and right there quickly when you needed them. Once again it’s a very well ran facility.
I just had to share this. My mom was a resident at this facility back when it was SKLD, and recently I ran into a nurse who took care of her during that time. You know how you never forget the nurses who truly make a difference the ones who show real compassion and empathy? That’s exactly who she was for my mom and our family. She made us feel at peace leaving our loved one in her care, and that means everything when your family member is in a nursing home. When she told me she is now the Director of Nursing, I couldn’t help but smile. Knowing the kind of nurse she was, I can only imagine the level of care and leadership she brings to the entire facility now. Shout out to D’Angela you truly deserve recognition for your hard work, compassion, and dedication. Families should feel confident trusting their loved ones in this facility.
I would like to thank Lavern for being an awesome Leader and Thank you for the referral. I'm kinda disappointed with the nurse for sending my sister off with a large bag of clothes and a wet bag of clothes and both bags belongs to someone else. Also, I'm suspicious about whether my sister was fed or not. When we arrived at the new facility she began to feel ill. Either she missed her breakfast, didn't get her medication or both. I couldn't find her patch she wears (for her memory). I've also noticed there were times when she would have 2 patches placed on her skin at the same time and the nurses were informed numerous times. Some days they would get my sister out of bed just to leave her unattended in the dining room while other residents were in the activity room. On March 19th the residents celebrated St Patrick's Day dressed in GREEN while my sister was in bed. If that's not rejection what is? Nurse BAO needs to be replaced
