I am deeply saddened to share that my family member’s experience at this facility has resulted in us now calling in hospice while dealing with a Stage 4 wound. After returning from a trip I found she had not had a bath in a week, room was not cleaned, sheets not changed, a cup with an old date was in the room, and bandages had not been changed nor dated. Our concerns regarding the lack of care were escalated to the Administrator, Director of Nursing, Business Manager, and Social Worker. While members of leadership apologized and stated that this was not the facility’s normal standard of care,…

While members of leadership apologized and stated that this was not the facility’s normal standard of care, one serious failure that contributes to an outcome of this magnitude is one too many. We were extremely concerned about the facility’s ability to appropriately manage wound care and the availability of qualified wound-care resources. Families trust a facility to recognize changes in a resident’s condition, provide appropriate care, and escalate concerns before they become this serious. I do want to recognize the second-shift Staff Supervisor and the physical therapist. They demonstrated compassion, concern, and genuine care for my family member during an incredibly difficult time. Their kindness is the reason I am giving this facility one star; otherwise, based on our experience, I would give it zero. I sincerely hope leadership takes this situation seriously and makes the necessary improvements so another resident and family do not have to experience what ours has.

I rate this place 5 stars My Mom was transferred to this facility from the hospital. When I got met there, I noticed three buttons on the outside entrance. Being my first time, I did not know which one to press. The receptionist snapped at me through the intercom. Speaking to me As if I should know how things work there. Once inside, there was no greeting I asked For directions to my mom’s room and I was giving the wrong directions. You also have to be buzzed out when you’re ready to leave. Don’t be in a hurry because you Might have to wait a while. The hallways Look OK, but the rooms Not so…

Once inside, there was no greeting I asked For directions to my mom’s room and I was giving the wrong directions. You also have to be buzzed out when you’re ready to leave. Don’t be in a hurry because you Might have to wait a while. The hallways Look OK, but the rooms Not so much. The wall vents were sticking out of the wall so anyone could get cut on the sharp edges. Very Old, Thin bedspread. No one came into the room to admit my mom or welcome her to the facility. She missed lunch so I tried to get her something to eat to no avail. By 9 PM she had not received her medication. My daughter addressed the nurse who dismissed her by mumbling under her breath. She needed to go to her car. I approached the same nurse the next morning to find out how my mom was during the night. The nurse responded by telling me how many people were admitted the night before and how busy she was. He was the same grouchy tone. My daughter received the night before. Needless to say, she never gave me an update. By the way, all of the nurses seem to be not very pleasant. I wanna occasion Housekeeping mopped the floor and left standing water on the floor Which was very slippery And dangerous. I addressed this with the supervisor of housekeeping, who was not concerned At all. My mom went to ER after 9 PM. The next afternoon before 2 PM, we called The office and were told that she had been discharged because she had been gone over 24 hours. Upon going to the facility to pick up her things, we were told she was discharged because she was admitted to the hospital. None of this makes sense because before We left for the hospital. The nurse asked me to sign a something for to hold the room. We could not get an explanation for that. I would not send my worst enemy to that place. I beg you not to go nor send your loved ones to this place. It should be closed down. This is probably why So many name changes.

My husband was here for a week and what a joke, he fell out of bed and they had No rails for bed because they tried to pass that off as not allowed because it is considered restrainint,!I guess hospitals and other facilities are allowed to constrain, there was NO chair in the room for the patient or visitor, my husband asked for one and they put it in there and I opened it and it collapsed and I went down before I even sat, I am not near as big as most patients let alone visitors, and HA HA on their “Chef” prepared meals, mind you I’m a retired chef so I know , wish I could share pictures ,…

A great team who cares

This worker named Da’Nasia is very sweet to my aunt Catherine H and I hear good things.

My father was sent to this facility during the last stage of a long rehab battle after heart surgery. The stay itself went well enough, although my father did have some issues with some of the staff not being overly friendly. Physical therapy was also good - their facilities are not the best but as good as you can expect for a lower budget nursing home. But do not expect anyone here to be communicative about their process, the patient's status or anything else. To understand where things are at with insurance, care, medications, the rehab process, expectations of outcomes, you have to actively set up meetings and ask those questions. Upon discharge, you get nothing except some paperwork. No explanations about wound care. No supplies or explanation of how to take care of a feeding tube - and the facility had not flushed his feeding tube or changed his wound dressings in over 36 hours. When you ask for a Med list - you get a giant packet of paper that has some med information buried in it, but the names and dosing instructions are truncated and therefore meaningless. And when you ask questions the discharge nurse talks to you like you're an idiot and most of their answers were "I don't know, I didn't do that portion of it but every one else did their jobs so it's all set, stop asking me". And the prescriptions themselves? I spent 6 hours on the phone the night of discharge trying to get the prescriptions called in that should have already been at the pharmacy waiting for us - but SKLD never called them in and argued with me about how late certain pharmacies were open when I was trying to get them changed to a 24 hours pharmacy. So my father who had been on a 4 month journey after heart surgery did not have any medications for over 24 hours after being discharged.
