My mom had her 4th stroke, and needed rehab to help strengthen her right side. Discharged from the hospital to this hell hole. We get there and she’s on a stretcher in the lobby for almost an hour before she gets a room. Then it was almost another hour before someone even came in to address that she was there. We then told them she needed a walker to use the bathroom (bathroom was in her room) and were told she couldn’t have one, nor could she have a wheelchair until she…

Do NOT LEAVE ANYONE HERE!!!! If I could give Zero stars I would!!! Disgusting, and looks nothing like the pictures online! My mom had her 4th stroke, and needed rehab to help strengthen her right side. Discharged from the hospital to this hell hole. We get there and she’s on a stretcher in the lobby for almost an hour before she gets a room. Then it was almost another hour before someone even came in to address that she was there. We then told them she needed a walker to use the bathroom (bathroom was in her room) and were told she couldn’t have one, nor could she have a wheelchair until she was evaluated by a physical therapist. Ok, great. Can we bring hers from home so she can use the bathroom? NOPE, they wanted to put her in a diaper until she saw the therapist. I’m not just talking about overnight, we came here on a Friday evening, and she wouldn’t see the therapist until MONDAY morning. So they wanted her to lay in that bed, using a diaper for 2 days? Nope, no thanks. We signed her out and took her home.
Four chaplains nursing home is a clean facility with caring staff and that truly listens to Residents. The team works hard to create a safe and comfortable environment every day. Thankto DON Kelly D for her leadership, support and dedication to both residents and staff. Highly recommended for compassionate long-term and rehab care.
My overall experience with this nursing home has been wonderful. The staff truly care about the residents and treat everyone with kindness, patience, and respect. The nurses and CNAs work very hard and always make sure residents are comfortable, safe, and well cared for. My overall experience with this nursing home has been wonderful. The staff truly care about the residents and treat everyone with kindness, patience, and respect. The nurses and CNAs work very hard and always make sure residents are comfortable, safe, and well cared for. A special thank you to Nurse Pat, Amarjit always meet me everyday in the morning and she is very kind and professional and cena Tommie and Kisha for their kindness, professionalism, and dedication. Their compassion and support truly make a difference every day. The therapy team is excellent and helps residents improve with encouragement and compassion. Special thanks to Tanveer Hussan helped me a lot during my rehab schedule days and The facility is clean, organized, and has a warm environment that feels welcoming to families and visitors. The food is good, and the activities keep residents engaged and happy. A special appreciation to the administration and Director of Nursing Kelly Dancy for their leadership and dedication to maintaining high-quality care. She always check on me on daily basis and others makes sure everyone is doing well, which truly shows her compassion and commitment. It is clear that teamwork and resident care are the top priorities here. I have an excellent experience here and truly appreciate the overall care and professionalism of the entire team. The staffing is well-organized, and it clearly shows in how smoothly everything runs on a daily basis. The therapy team is outstanding—very attentive, knowledgeable, and genuinely focused on helping residents improve and feel comfortable. The food is surprisingly good, with a nice variety and quality that makes a difference in everyday life. The nurses and CNAs are compassionate, hardworking, and always willing to go the extra mile to ensure residents feel safe and cared for. A special thank you to Dietitian Rachel and t the Director of Nursing Kelly Dancy for exceptional leadership. Their dedication, support, and commitment to high standards truly reflect in the quality of care provided. My mom has been here since 2024. She likes it... The staff knows her by name, she feels safe and cared for. Residents seem to be active and happy. I have direct contact to administrators. Location is perfect. The staff knows me. Amarjit is super helpful when it comes to providing support for resources. I have suggested 4 Chaplins to friends for their loved ones. Visit and See for yourself.
Overall, the care is good. Most of the staff are very friendly. Jasmine and Shawntay are really awesome nurses! Toyia, in Admissions, offers great customer service, she has been tremendously helpful. Denise and Diane truly care about the residents! Bernie in activities is the best. The food is good. The Physical Therapy staff are very helpful and want residents to succeed!
I am in 4 chaplains nursing home for physical therapy after about of pneumonia. I am getting excellent care, everyone is very nice and treats me very well. They are very kind and meet my needs. Management is extraordinary, they are helping and pull well to put the team together and special thanks to DON Kelly Dancy. My father was here many years ago and received a great care , I am receiving 💕💕💕 I highly recommend this place for your loved ones care .
