I would give a zero star review if I could, however in order to leave a review I have to select one star. DO NOT TAKE YOUR LOVED ONE TO THIS CENTER IF YOU WANT THIER WISHES REGARDING DNR to be honored. Our family member was admitted Friday with his POA signing the DNR, the facility doctor did not sign the order for this and on Saturday our family member was who was on Hospice with signed DNR paperwork, missing the physician signature and was Intubated at the hospital. When family was notified we immediately requested this be removed. They were sent back to Optalis where he was unresponsive and the facility ignored the POA, who was present and was notifying staff again they wished to be a DNR. The POA repeatedly notified them that the signed DNR form from Friday was in the patients chart. That the patient was a DNR and there should be no life sustaining steps taken. The staff stated they could not honor the DNR because it was not signed by the physician. The police were called, EMS were called and unfortunately our FOUR TIME IRAQ MARINE WAR VETERAN died in the most undignified manner.
I love my experience here. I love my girl Britney she is very help to me with any and everything. The activity department has two very outstanding girls. This place is under new leadership and I see the change and very please.. I will be transitioning out Friday and if i ever need a rehab again I will be back at Canton . Thank you all.
My 97 year old dad was there 8+ weeks for physical and occupational rehabilitation following a bout with COVID 19 and 2 week hospitalization. While the CNAs and nurses were generally pleasant and professional, staff turnover was extremely high, staff morale was horrible and it appeared to be under staffed. I was there with my dad every day spending hours and only saw his physician once. The shower was never cleaned and my dad was never showered the entire time he was there. He got C-Diff while there and was discharged to come home by the facility’s physician even though they (RN and doctor)…

He got C-Diff while there and was discharged to come home by the facility’s physician even though they (RN and doctor) knew he was very sick. Within hours of being home he was transported by ambulance to the ER, was admitted and died 10 hours later. He was a vibrant, mentally sharp person despite his age even after recovering from COVID. I do not recommend sending your loved ones here.
I haven't even been here a day, but the night was horrible due to one young girl that didn't know how to care for people. I was very fortunate when I met Tiffany. She made my stay SO much better 💕 💓. And my ladies in PT really helped me and cared. TY Jenn, Barbara, Gabriella, I am forgetting a lot of names but they were phenomenal ✨️ . TY to all
I give this place five stars of 10 out of 10 the place was very clean. It smelled nice. The staff was very respectful. The food was great. I really enjoyed my roommate that I shared a room with my experience was great. I really don’t have anything bad to say about the place. I recommend this for anyone that needs to be in a rehab center or needs to get help in the future, but my best overall experience was the intake manager. Brittany runs the first day. I met her when she did my intake. She was so nice respectful understanding there’s nothing that I needed that she did not help me with her smile brighten my day she come and checked on me every day to see if I needed anything to help me heal and recover, as well as on my journey to go back home. She was the best part of me being there thank you Brittany for everything that you did and for making me feel better and like I was at home.
My great aunt has a private room, which has been great for her comfort and privacy. I’ve met some really awesome people during our visits. We did have a small issue, but it was taken care of quickly, which I really appreciated. A special shoutout to Nurse Beck, who was very helpful and kind. The woman in admissions was also attentive, welcoming, and made the process much easier for our family. The food could be a little better, but overall it’s a nice facility with friendly people who genuinely seem to care. I’m very satisfied with our experience and would recommend it to others. 😊
I recently visited this nursing home both as a family member inquiring about care options and as a delivery driver, and I was genuinely impressed with the experience. The facility is exceptionally clean, well-maintained, and has a warm, welcoming atmosphere. During my visit, Britney was incredibly kind, professional, and helpful. She took the time to answer my questions, provide information, and made me feel comfortable throughout the process. Her compassion and dedication truly stood out. What I appreciated most was how personal and attentive the staff are with residents and visitors alike. It’s clear that they take pride in creating a caring environment where people feel valued and respected. Based on my experiences, I would highly recommend this facility to anyone seeking quality care for a loved one.
My family member was in this location for a few weeks and it was terrible. They do have some workers that care but few and far between. They are short staffed. Some were very rude and even told my family member to shut up and let her do her job. I could list many instances. Begging someone to come and help go to the bathroom. Wrong pills delivered to the room and they had to hurry and take them back before they were taken. The food was terrible but the main concern I had was how the family member was treated. It's a sad situation when you feel helpless. The rehab center seemed to be professional but it doesnt make up for the insensitive people that work their.
