The floors in the facility appeared very dirty and sticky. The waiting room sofa was extremely dirty and did not look like it was being properly cleaned or maintained. There was also a strong unpleasant smell, including urine and gas odors, which made the environment feel uncomfortable. I visited The Orchards at Warren today while my family member was being admitted for rehabilitation, and unfortunately my first impression was very disappointing. The floors in the facility appeared very dirty and sticky. The waiting room sofa was extremely dirty and did not look like it was being properly cleaned or maintained. There was also a strong unpleasant smell, including urine and gas odors, which made the environment feel uncomfortable. For a rehabilitation facility where patients are recovering from serious illnesses such as strokes, cleanliness, hygiene, and a caring environment are extremely important. We expected a place where patients would feel safe, respected, and well cared for, but our first experience did not meet those expectations. I hope management takes these concerns seriously and improves cleanliness, maintenance, and the overall environment for patients and their families. This review is based on my personal experience during my visit.
A family like atmosphere, a great place for LTC, STC, & Rehab ❤️

This is a g place I would recommend it
i was deeply concern about the conditions at this care facility. She reported being largely isolated, with strict visitor restrictions that seemingly only exempted her granddaughter. My friend described feeling like a prisoner, trapped in an environment that neglected her basic needs. She had been residing there for months and was denied regular personal hygiene. The staff's apparent lack of care was evident in their reluctance to assist her with bathing, often opting for bed baths instead of helping her access shower facilities. Most disturbingly, her medical condition had visibly deteriorated, with her bedsiores growing larger during her stay, suggesting significant medical negligence. Please do not use this place!
The building is old but was clean. The staff was very nice and helpful. Food was ok for the most part.
The staff and professionalism are God send. I live in FL and my Dad received rehab care from this facility. Very clean, friendly staff, and the professionalism are some attributes. Adi, Tameka, Antwan, Keisha, Shania, Marla, Naz, Piper, Christina, Rhonda, Michael, Tiffany, are truly assets to this facility ❤️ Thank you for providing quality care for my Dad. You all provide services that are priceless.
