When you call the Billing Manager to inquire about the bill full of unauthorized charges, they refuse to make any adjustments, and instead, ask you to set up a payment arrangement for the $4700 worth of unauthorized charges that they sent you. Fraudulent Billing.
Amazing team of nurses who provide exceptional care.
Walked in with 2 witnesses to find loved one NUDE from waist down. Nurses REFUSED TO CLOTHE HIM! Found out it was ongoing pattern of abuse & neglect. He was afraid to say anything for fear they would kill him. Nurse in charge of care plans has hung up on me twice even though l was NOT RUDE. One time she hung up because l would not pray with her over the phone!! She knows nothing of my religious affiliation. Even though responsible for his care plan, she says he will not have therapy for months! He will die before then. She will have blood on her hands, as will others at Optalis. Filed police…

My grandmother came for Rehab and had a wonderful experience at SKLD Muskegon. Friendly and happy staff who were always helpful. There was no smell when you came in. Very nice room close to the therapy gym. Kim the nurse was always kind and very helpful. Overall a 5 star building.
SKLD Muskegon is a great facility to bring your loved one to. They treated my uncle with dignity and respect. The Therapists, Nurses, and CENAs were attentive to all of his needs and the social workers were very supportive during the entire stay ensuring a smooth transition back home.
I visited SLKD to see a family member. I am a CNA and offered to assist with the family member to take a load off of the staff. Libby (Elizabeth) was extremely generous, kind, understanding, and put the safety of my family member as her top priority. She greeted us with a huge smile and a happy attitude. She watched my transfer and talked me through it to ensure my family members safety. Libby was/is an amazing person to interact with and is so amazing at her job. I trust her with my family member.
